THE SECRETAGOGUES COMPARED

Sermorelin vs. HGH — How They Differ and Who Each Is For

By G1 Peptides · Medically reviewed by the G1 Peptides clinical team · Updated October 1, 2026 · 2 min read

THE SHORT ANSWER

HGH is growth hormone itself, injected directly. Sermorelin is a peptide that signals the pituitary gland to release more of the body's own growth hormone. Sermorelin keeps the body's natural feedback intact, so its effect is milder and self-limiting. HGH is approved only for specific medical conditions, and prescribing it for anti-aging is prohibited in the United States.

How each one works

The pituitary gland releases growth hormone in pulses, mostly at night. Two signals control it: GHRH tells the pituitary to release, and somatostatin tells it to stop.

Sermorelin is the active part of GHRH. It prompts a pulse of the body's own growth hormone. When levels rise, the body's normal feedback slows further release, so the effect is self-limiting (Walker 2006).

HGH is the hormone itself. It raises levels regardless of what the pituitary is doing, and the feedback loop cannot hold it back. That makes its effect stronger and more predictable, and it is also why overdosing is possible.

Effects

Both raise IGF-1, the hormone that carries out most of growth hormone's effects. Studies of sermorelin in adults report modest changes in body composition and better sleep, with IGF-1 staying within the normal range. HGH produces larger changes in lean mass and fat, at the cost of more side effects.

Sermorelin needs a pituitary that still responds. If the gland cannot produce growth hormone, a secretagogue will not help.

Side effects

  • Sermorelin: flushing, headache, and injection-site reactions are the most reported. Because feedback is preserved, the effects of excess growth hormone are less likely.
  • HGH: fluid retention, joint pain, carpal tunnel symptoms and raised blood sugar are well documented, and they increase with dose.

Neither should be used by someone with an active cancer. See who should not use peptides.

HGH is approved for conditions such as growth hormone deficiency. Under United States federal law it cannot be prescribed or distributed for uses outside those approvals, which rules out anti-aging and body-composition use.

Sermorelin was once sold as an FDA-approved product. The brand was withdrawn for commercial reasons, and it is now supplied by compounding pharmacies on prescription.

Other options

Sermorelin is one of several secretagogues. Sermorelin vs. CJC-1295 vs. ipamorelin compares the alternatives, and the sermorelin profile has the details on dosing and half-life.

FREQUENTLY ASKED

Is sermorelin the same as HGH?

No. HGH is growth hormone. Sermorelin is a fragment of growth hormone-releasing hormone, the signal that tells the pituitary to release growth hormone. Sermorelin raises your own production; HGH replaces it.

Is sermorelin safer than HGH?

Sermorelin works through the body's own feedback loop, which limits how high growth hormone can go. That is the main safety argument for it. It has not been compared with HGH in a long-term safety trial, so it is more accurate to say the risks are different and generally milder.

Why is sermorelin prescribed instead of HGH?

In the United States, HGH can only be prescribed for a short list of approved conditions, and prescribing it for anti-aging or body composition is prohibited by federal law. Sermorelin is not subject to that restriction and is supplied by compounding pharmacies.

Does sermorelin work as well as HGH?

Its effect is smaller. Sermorelin raises growth hormone and IGF-1 within the normal range, and it depends on a pituitary that still responds. HGH produces larger and more predictable increases, with more side effects.
